Production Tracking Form
Reguired before CTV facilities or equipment are used. Also used for van shoots. You can fill it out and then print it from Acrobat Reader. This form is also the compliance form. Upon completion of your project, you request your form back and complete the right side of the the form.

Outside Production Form
Programs that are produced outside of CTV must use this form when submitting their program for broadcast. (If the production did not involve CTV equipment or facility and was not produced within our ten city area, use this form.)

Videotape Dub Request Form
Use this form to being the process of requesting a duplicate of a video.

Model Release Form
For producers to use with their talent. It is to authorize the reproduction, copyright, exhibition, broadcast, cablecast and for distribution of said videotape without limitation for the purpose of public access programming.

Parental Statement of Compliance
Minors (under 18) must have their parents fill out this form to check out the equipment.

Premise Release
For producers to use with filming on private property. To authorize the reproduction, copyright, exhibition, broadcast, cablecast and for distribution of said videotape without limitation for the purpose of public access cable programming.

Master Mailing
People who would like to be on our volunteer listing can use this form.

Program Release
Authorization for CTV to make copies for a third party.
